Prognostic Significance of Tumor Size in Curatively Resected Gastric Cancer

Abstract
Retrospective studies using multivariate analysis of clinical and pathological features in gastric cancer have identified a number of independent prognostic factors; howerer; the prognostic significance of tumor size remains unclear. The aim of this study was to determine the significance of tumor size as prognostic factor; by reviewing gastric cancer cases treated with curative resection at Dongsan Hospital. Between January 1996 and December 2001; 1;132 patients who underwent gastric resection for gastric cancer were reviewed. The sizes of tumors were the measurement made by fresh resected specimens. The patients were divided into two groups: 871 patients with tumors of less than 5 cm in diameter; and 261 patients with tumors of more than 5 cm in diameter. In these two groups; there were statistical differences in macroscopic type; vascular invasion; nerve invasion; survival rate; relapse free survival rate and the level of tumor markers. The 5-year survival rate and 5-year relapse free survival rate in the groups with tumors of less and more than 5 cm diameter; were 90.2% and 85.1%; and 72.7% and 62.5%; respectively. Multivariate Cox regression analysis revealed that tumor size was an independent prognostic factor to predict total survival and relapse free survival in curatively resected gastric cancer.
